Chris Hart

Transportation Safety Expert & Leader

Chris Hart is a lawyer, former government official, pilot and transportation safety expert. He spent decades as a transportation safety regulator at organizations like the Federal Aviation Administration, National Highway Traffic Safety Administration and National Transportation Safety Board. Hart developed a passion for fostering collaboration to improve safety and productivity and has leveraged lessons learned from the celebrated Commercial Aviation Safety Team to make our world safer for everyone.

Rebecca Cady

Chief Risk Officer at Children’s National Health System

Rebecca Cady is Vice President, Chief Risk Officer and Deputy General Counsel at Children’s National Hospital in Washington, D.C. Cady served as President of the American Society for Healthcare Risk Management in 2023. She has published extensively in the areas of nursing and the law and is a frequent lecturer to healthcare providers.

Steve Burrows

Director of HBO documentary, “Bleed Out”

Steve Burrows is the director of the critically acclaimed HBO documentary, “Bleed Out,” and a founding member of Patients for Patient Safety US. In 2009, Burrows dropped out of show business to take care of his mother, who had gone in for a routine partial hip replacement and came out in a coma with permanent brain damage. He documented these years, highlighting his journey for medical and legal justice.

Colonel Steven L. Coffee

Co-founder of Patients for Patient Safety US

Col. Coffee is a career military officer and a founding member of the Patients for Patient Safety US. His passion for patient advocacy arose following the misdiagnosis of a metabolic condition in his newborn son, causing him to receive a liver transplant at eight weeks old. Coffee serves as a charter member and the inaugural community Chair of the MedStar Georgetown Patient and Family Advisory Council for Quality and Safety.

Carole Hemmelgarn

Patient Safety Advocate & Leader

Carole Hemmelgarn is the Director of the Executive Master’s program for Clinical Quality, Safety & Leadership at Georgetown University, and the Senior Director of Education for the MedStar Institute for Quality & Safety. Carole is a leading patient safety advocate focused on improving transparency and communication after medical harm happens.

Tim McDonald, MD, JD

RLDatix Chief Patient Safety and Risk Officer

Dr. Tim McDonald is a physician-attorney who has assisted more than 200 hospitals and health systems to implement a culture of “normalized compassionate honesty” combined with “fair and accountable culture” transformation. Before joining RLDatix, Dr. McDonald served as the president of the Center for Open and Honest Communication at the MedStar Institute for Quality and Safety and a Professor of Law at Loyola University – Chicago.

Barbara Staruk is a product management executive with nearly 25 years of experience in mission-critical healthcare software.
